You will need to have it reduced when copied. - We MUST have the entire year’s report card. - We MUST have copy of all 3 trimester or FULL YEAR Reported 2) Participant Contract and Parental Consent Form - Signed by both parent and player/cheerleader 3) Clear COPY of Birth Certificate - Must have state seal 4) Clear COPY of current Utility Bill - Must show name, address, zip, & current date 5) Physical Form - MUST be completed on the Palomar Conference Physical Examination form. - We MUST have the original copy. - The examining Doctor MUST sign, stamp, and date the form. |
