TO BE ELIGIBLE FOR THE 2012-2013 SAUK PRAIRIE YOUTH BOY’S BASKETBALL TEAMS (4^th, 5^TH, 6^TH, 7^TH, and 8^th Grade), THIS COMPLETED FORM MUST BE RECEIVED BY:  Edie Radl, 6776 Cty Hwy KP, Sauk City, WI  53583 -- NO LATER THAN September 15, 2012!   Late registration fee of $25.00.
