Documents 1 through 4 will be used for the 2011 season.  All players use the Club Membership Form and the Club Medical Consent Form.  Players 17 years old or younger use the Minor Waiver and Release.  Players 18 or older use the Adult Waiver and Release.  Every player needs to turn in a Membership Form, Medical Consent, and Waiver, plus a dues check (amount determined on the Membership Form):
