Mid-Dakota Baseball Academy
2012 Fall League

 

The Mid-Dakota Baseball Academy will be conducting its 1st annual Fall Baseball League. The league will be held on select Saturdays and Sundays in September and early October. The purpose of the Fall Baseball League is to give athletes the opportunity to continue their development in baseball.

Important League Info:

• Sign up individually or get your team to sign up!
• Teams will consist of 10-11 players.
• Dates: Sundays, September 2nd - October 7th and Saturdays, September 15th and 22nd
• Who: high school students grades 9-12 (8th graders can sign up with their coaches recommendation).
• Where: Mt. Vernon, SD City Baseball Park
• Cost: $25.00 (includes team t-shirt)

Release & Indemnity Agreement

To Mid-Dakota Baseball Academy, City of Mt. Vernon and the Mt. Vernon Baseball Association:
(the City of Mt. Vernon and the Mt. Vernon Baseball Association does not endorse/sponsor the MDBA)

We (or I) hereby request that you accept the enrollment of (player’s name)__________________________________________ to the Mid-Dakota Baseball Academy. With your consideration of the acceptance of the application, we (or I) hereby release MDBA, the City of Mt. Vernon and the Mt. Vernon Baseball Association and all its employees from all claims on account of any injury, which may be sustained by our (or my) student while attending/participating in the MDBA's Fall Baseball League. And we (or I) agree to indemnify MDBA, the City of Mt. Vernon and the Mt. Vernon Baseball Association and its employees for any claim that may hereafter be presented by our (or my) player as a result of any such injuries.
